Aim of the study: Type I allergies are believed to he involved in chronic sinusitis., i.e. allergic sinusitis. The clinical effect of a topical steroid delivered via YAMIK sinus catheter was evaluated for treatment of allergic sinusitis.

Materials and Methods: The subjects were 37 patients (mean age, 30.1 years; range, 8-63 years) with sinusitis accompanying perennial allergic rhinitis. None of these patients had purulent or mucopurulent nasal discharge. Twenty-five patients had bilateral sinusitis and 12 patients has unilateral sinusitis. Forty four sinuses (one side in 30 patients and both side in 7 patients) were inoculated with 64 mcg/ml of fluticason propionate after evacuation of sinus effusion, both via YAMIK sinus catheter. This treatment was performed once per week for 4 weeks. Another IS sinuses (one side) were treated only evacuation of sinus effusion as controls. All patients were given an oral-antiallergic drug for the treatment of allergic rhinitis. Efficacy was determined mainly by comparing X-ray findings before and after treatment

Results: Improvement in the sinuses treated with flulicasone propionate via YAMIK sin catheter was observed in 55%, whereas improvement under control conditions was seen only 33%. The amount of sinus effusion obtained in the fourth evacuation decreased compared to that obtained in the first evacuation. The mterlcukin-4 concentration was lower in the effusion obtained in the fourth evacuation than that obtained in the first evacuation.

Conclusion; These results suggests that inoculation of a potent topical steroid via YAMIK sinus catheter is useful for the treatment of allergic sinusitis accompanying perennial allergic rhinitis.
